Which support should you explore?
Different types of support address different parts of daily life. Choose the situation that most closely reflects what you want to discuss.
Support with daily life at home

You do not need to have every detail ready. A short summary of the support being requested can help the first conversation stay focused.
- The participant’s preferred name and age group.
- Your relationship to the participant if you are enquiring for someone else.
- The suburb or general location where support is needed.
- The main goals, routines or activities the participant wants support with.
- Whether support is mainly needed at home, in the community or through nursing care.
- Preferred days, times or frequency, where these are already known.
- Communication, cultural or personal preferences that are important to the participant.
- The best phone number, email address and preferred contact method.
Protect sensitive information. A general enquiry usually needs only a clear overview. Avoid sending detailed medical records or highly sensitive documents until an appropriate way to provide them has been discussed.
Useful questions to ask a support provider
Clear questions can help participants, families and representatives understand whether a provider may be suitable before entering a service agreement.
- Does the requested support fit the provider’s confirmed service scope?
- How will the participant’s goals, routines, preferences and communication needs be discussed?
- How are location, staffing and service availability confirmed?
- Who should be contacted when support arrangements need to change?
- What information will be collected, and how will privacy be protected?
- What will the service agreement explain about responsibilities, schedules, fees and changes?
- Which supports or arrangements are not included?
- How can feedback, concerns or complaints be raised?
Important distinctions to understand
Knowing what is and is not included can prevent confusion and help you direct an enquiry to the most relevant service.
SIL support is different from housing
Supported Independent Living relates to support delivered in a participant’s home. Australian Inclusion Care Solution can discuss support staffing in an existing SIL or shared living arrangement but does not provide the house, accommodation or vacancies.
Community nursing is not emergency care
Community Nursing Care is planned according to individual needs, suitable nursing availability and an agreed service arrangement. It is not an emergency medical service. Call 000 in an immediate or life-threatening emergency.
Transport arrangements require discussion
Transport-related support may form part of access to appointments or community activities. The exact arrangement depends on the participant’s needs, available staff and the agreed service arrangement. Dedicated or guaranteed transport is not offered.
Availability is considered individually
Support depends on the participant’s needs, location, suitable staff availability and service agreement. An enquiry does not guarantee acceptance, immediate commencement or a particular schedule.
Preparing a clear referral or enquiry
Parents, family members, support coordinators, plan managers, case managers, hospitals, schools and allied health professionals may contact Australian Inclusion Care Solution about a participant’s support needs.
- Your name, role, organisation and contact details.
- The participant’s consent or your authority to make the enquiry, where applicable.
- A concise description of the support being requested.
- The participant’s general location and preferred service setting.
- Relevant communication, cultural, routine or access considerations.
- Known timing needs without assuming immediate service commencement.
- Other professionals who may need to be involved, where appropriate and authorised.
- The preferred person and method for follow-up communication.
Australian Inclusion Care Solution may work alongside families, agencies and relevant professionals where appropriate. A formal partnership or coordinated arrangement should not be assumed until it has been discussed and agreed.
